*Applicants should make changes to Biographical/Demographical information via MyCalPoly in the "Peoplesoft Admissions Self Service Channel."
** All test scores need to be sent electronically from the testing agency or via CSUMentor.

- Your full legal name (as it appears on your application for admission)
- Your complete mailing address
- Your change request including what was originally submitted
Be Advised: All submitted changes are subject to approval. Changes not listed above will not be processed. When you submit your application, you are certifying all information is accurate and complete. |
