(1) One 'official' copy of high school transcripts to include your date of graduation. Failure to provide a final high school transcript will affect your enrollment in the future. Do not send your high school transcript UNTIL you have graduated and the transcript includes your graduation date (usually May or June) unless otherwise instructed by the Admissions.

(2) If you have taken courses at a community college or university, have transcripts of that college work sent to Cal Poly directly from the institution AFTER you have completed the coursework. Do not send a ' work-in-progress' listing of college courses in which you are currently enrolled.

(3) Have ACT or SAT I scores sent to Cal Poly if you have not already done so or if they do not appear on your high school transcript. Cal Poly requires that test scores be sent electronically from the testing agency (or via the CSUMentor website for SAT). Paper test scores are not accepted.
