Please fill out the following form and click the "Submit" button to schedule an appointment with your Academic Progress Counselor. Complete all fields or your request may be delayed or not processed.
Submit your request only once. Your Academic Progress Counselor will contact you to finalize your appointment time. (Please be patient.)
Once the appointment has been set, bring a copy of your Student ID card, Degree Progress Report, and any other pertinent paperwork.
